The 10th National Congress of the Italian Association for Revision Arthroplasty, AIR, one of Italy’s leading events dedicated to prosthetic revision surgery, chaired by Filippo Castoldi, Fabio D’Angelo and Giuseppe Solarino, is currently taking place in Riccione.

The event, hosted at the Palazzo dei Congressi Palariccione on 16 and 17 April, brings together specialists from numerous Italian centres for a scientific discussion on the new frontiers of hip, knee and shoulder revision arthroplasty

For the University of Insubria, the presence of Professor Fabio D’Angelo among the congress chairs confirms the University’s contribution in a highly complex clinical and surgical field. D’Angelo also sits on the AIR Board of Directors 2024-2026 as Secretary, demonstrating stable and authoritative participation within AIR.

The scientific rationale of the congress underlines how revision surgery is now one of the most pressing challenges in orthopaedics, in light of increasing population longevity, the growing number of primary prosthetic implants and patients’ ever higher functional expectations. In this context, the AIR congress addresses the main issues in prosthetic revision in a cross-cutting way: biomechanics, implant selection, optimisation of surgical technique, treatment of periprosthetic infections, management of bone loss, joint instability and complications. Ample space is also devoted to new technologies, from robotic surgery to 3D planning, through to applications of artificial intelligence and regenerative medicine

The first day opened with initial greetings from the three chairs and a first session dedicated to free-topic oral presentations. The programme then offered in-depth sessions on pharmacological management between diagnosis and reoperation, with a focus on pain therapy, osteotrophic therapy, biophysical therapy and prevention of deep vein thrombosis. A specific session was then dedicated to the relationship between innovation and revision surgery, with talks on AI & machine learning, virtual and augmented reality, CT and MRI, signalling a field increasingly oriented towards the integration of surgical expertise and advanced tools for clinical and decision-making support.

During the day, there were also sessions dedicated to increasing constraint in revisions, the management of bone defects, the role of soft tissues and so-called “minimal” revisions, with a scientific programme designed to compare experiences, approaches and solutions across three major joint areas. The day will conclude with the opening ceremony of the congress and greetings from the presidents of the main scientific societies in the sector.

The scientific programme also includes a contribution directly linking clinical activity in Varese and the University of Insubria, authored, among others, by Andrea Pautasso and Fabio D’Angelo: “The project is dedicated to the definitive spacer in the treatment of periprosthetic shoulder infections and to comparison with the two-stage prosthetic revision method,” explains Professor D’Angelo. “The work involves ASST Sette Laghi – Ospedale di Circolo e Fondazione Macchi in Varese and the Department of Biotechnology and Life Sciences of the University of Insubria, highlighting the contribution of the local area and the University in a particularly significant field of research and care.”

The congress also features sessions dedicated, among other topics, to revision implants in sequelae, implants in tumours, custom-made implants, the management of PJI, and awards for the best oral presentations and best e-posters. In this context, the participation of the University of Insubria forms part of a high-level scientific discussion aimed at improving the clinical and functional outcomes of patients undergoing prosthetic revision surgery.
