The Erasmus+ Study students are kindly requested to use these forms, according to the period:

• before the mobility (to be prepared and delivered before the departure)
• during the mobility (to be used for changes or certificates while abroad)
• after the mobility (to be prepared and delivered after the return back to Italy)

Before the mobility

• Learning Agreement
• Grant data

During the mobility

• Statement period form
• Learning Agreement changes
• Request for Extension of the Erasmus Period

After the mobility

• Recognition form
• Recognition form (Medicine and Surgery students only)

The Erasmus+ Traineeship students are kindly requested to use these forms, according to the period:
