40 years old, by multiple basaliomas of the trunk. Considering her
relative young age, after surgery the patient was genetically studied:
the most common chromosomical and genetical alterations responsible
of well-known basaliomatosis syndromes could not be detected.

The Authors present a new technique used in postmastectomy breast
reconstruction by a tissue expander implantation: we created two pockets,
a submuscular one to accommodate the expandable device and a second
one placed between muscle and skin in order to create a mobile layer
in the chest wall, helping soft tissues to expand.

We present a case in which a 75-year-old woman, affected by an istyocitoma,
treated with radiotherapy and chemotherapy, was sent to our attention
because the port-a-cath she had in the right mammary region was nearly
extruded; a modified Dufourmentel flap was sculptured including a
part of the capsula of the port-a-cath; the device could be use the
day after surgery.

Lichen planus is a cell mediated immune response characterized by
a papular eruption with a mucous or a cutaneous localization. Malignant
transformation of lichen planus is more frequent in the mucous type
than in the cutaneous type. The authors present a case of a woman
in which a squamous cell carcinoma developed on a previous lesion
of hypertrophic lichen on a leg.
25) Idiopathic circumscripta calcinosis cutis of the knee: a case

Idiopathic circumscripta calcinosis cutis is a rare disease, typically
involving the extremities, associated to trauma and sclerodermia.
Treatment is medical or surgical, but benefits are limited and temporary.
The Authors present the atypical case of a 46-year-old woman, affected
by idiopathic circumscripta calcinosis cutis of the knee, successfully
treated by surgical removal.

The Medial Sural Artery Perforators. Anatomic Basis For a Surgical

The AA present an anatomical study on medial sural artery perforator
flaps performed on 20 legs of fresh cadavers. The average dimension
of flaps were 12.9 x 7.9 cm; the 38 perforators were muscolocutaneous
and were found between 7 and 18 cm from the popliteal crease; the
34.2% of perforators arose on the midline of the medial head of gastrocnemius
muscle and their average caliber was 0.5 mm before entering the fascia;
66% of perforators originated from the lateral branch of the medial
sural artery and 34% from the medial branch. This study improves the
anatomic knowledge of the medial calf region, making flap planning
and sculpturing easier.

The authors present a new method of mandibular reconstruction with
frozen autologous mandibular bone. Vascular supply to the neomandible
is ensured by the periosteal layer of a microvascular radial periosteal
fasciocutaneous free flap. We decribe two cases of oral carcinoma
involving the mandible. This technique allows good functional and
aesthetic results, avoiding complications related to the use of composite
free flaps.

The Authors present their technique for the esthetique treatment of
depressed scar after tracheostomy; in a day-surgery admission we used
the the classic deepithelialized bilateral triangular flaps composed
by dermis, subcutaneous fat and fascia with 3 everting vertical absorbable
mattress stitches under the flaps: after a 6 months follow-up the
scar was still flat.30) A Modified inverted omega incision for surgical
treatment of gynecomastia.
